Job Title: RMNCAEH Quality of Care (QoC) Mentor

The RMNCAEH Quality of Care (QoC) Mentor will provide routine on-site mentoring, coaching, and technical assistance to health facilities to strengthen the quality of Reproductive, Maternal, Newborn, Child, Adolescent Health and Nutrition (RMNCAH+N) services. The mentor will support facilities to identify quality gaps through data-driven approaches, strengthen adherence to national clinical standards and guidelines, institutionalize quality improvement processes, and enhance service delivery outcomes. They will conduct quarterly QoC assessment across supported facilities using the 8 standards of car.

Goal

To improve the quality of RMNCAEH and Nutrition services through continuous mentoring, coaching, quality improvement support, and data-driven decision-making at supported health facilities.

Specific Objectives

Strengthen health worker competencies in the delivery of high-impact RMNCAEH and Nutrition interventions. Improve the use of service delivery data for identification and resolution of quality gaps. Strengthen Infection Prevention and Control (IPC) practices within supported facilities. Improve referral and feedback mechanisms for maternal, newborn, child, adolescent health, and nutrition services. Establish, revitalize, and strengthen functional Quality Improvement Teams (QITs). Institutionalize continuous quality improvement using recognized QI methodologies and tools.

Routine Mentoring and Coaching of health care workers

Conduct regular facility visits (20 days per month) to provide on-site mentoring and coaching to healthcare providers. Support providers to effectively implement high-impact interventions in maternal, newborn, child, and adolescent health, including nutrition services Reinforce compliance with national clinical protocols, standards, and guidelines. Observe service delivery practices and provide constructive feedback for performance improvement. Facilitate skills transfer through demonstration, bedside coaching, and case reviews. Data-Driven Quality Improvement Support

Review facility performance data to identify service delivery and quality improvement gaps. Support health workers to routinely analyze data from registers, patient records, tally sheets, DHIS2 reports, partographs, maternity registers, child health registers, nutrition registers, and other source documents. Guide facility teams to use data for evidence-based decision-making and planning. Track priority quality indicators and support facilities to improve performance. Quality Improvement (QI) Processes

Support facilities to identify priority quality problems using routine service delivery data. Facilitate root cause analysis using quality improvement tools including:Five Whys Analysis, Fishbone (Ishikawa) Diagram and Process Mapping Support facility teams to prioritize identified causes and develop change ideas. Guide facility teams in the application of the Plan-Do-Study-Act (PDSA) cycle to test and implement solutions. Monitor progress of improvement projects and document lessons learned and best practices. Strengthening Quality Improvement Teams (QITs)

Support facilities to inaugurate or revitalize Quality Improvement Teams (QITs). Orient QIT members on their roles and responsibilities. Facilitate the development of facility-specific quality improvement plans. Support QITs to conduct monthly meetings and maintain meeting records. Review QIT action plans and monitor implementation of agreed actions. Promote multidisciplinary participation and ownership of quality improvement activities. Maternal, Newborn, Child, Adolescent Health and Nutrition Support

Provide technical guidance on quality service delivery across RMNCAEH and Nutrition service areas. Support implementation of interventions aimed at reducing maternal, newborn, and child morbidity and mortality. Strengthen nutrition assessment, counseling, referral, and follow-up systems. Promote adherence to standards for ANC, labor and delivery, postnatal care, newborn care, child health, adolescent health, and nutrition services. Referral System Strengthening

Provide technical support to strengthen two-way referral systems. Ensure proper documentation of referrals and feedback mechanisms between referring and receiving facilities. Review referral registers and referral outcomes during mentoring visits. Support facilities to identify and address referral bottlenecks affecting service quality and continuity of care. Infection Prevention and Control (IPC)

Assess compliance with IPC standards and protocols. Provide practical coaching on hand hygiene, environmental cleaning, decontamination, sterilization, and waste management. Support facilities to establish and maintain proper healthcare waste segregation and disposal practices. Identify IPC gaps and support implementation of corrective actions. Promote availability and appropriate use of IPC commodities and personal protective equipment. Documentation, Reporting and Follow-Up

Develop mentoring visit reports with documented observations, recommendations, and action points. Maintain records of mentorship sessions, QI activities, and facility performance improvements. Share feedback with facility management and relevant stakeholders. Monitor implementation of agreed action plans during subsequent visits. Highlight best practices, success stories, lessons learned, and persistent challenges requiring escalation.

Deliverables

Monthly facility mentorship reports. Facility-specific quality improvement projects. Root cause analysis reports for identified quality gap and improvement tracking records. Monthly QIT meeting reports and attendance records. Quarterly QoC Assessment Report

Required Qualifications 

The RMNCAEH Quality of Care (QoC) Mentor shall possess the following minimum qualifications, professional experience, technical competencies, and personal attributes:

Bachelor’s Degree in nursing, Midwifery, Medicine, Public Health, Community Health, or another relevant health-related discipline from a recognized institution. A master’s degree in public health, Nursing, Midwifery, Health Services Management, or a related field will be an added advantage. Valid professional registration and current practicing licence with the relevant regulatory body, where applicable.

At least 5 years of relevant professional experience in RMNCAEH, primary healthcare, quality improvement, or health systems strengthening. At least 3 years of demonstrated experience providing facility-level mentoring, coaching, supportive supervision, or technical assistance to healthcare providers. Demonstrated experience working with primary healthcare facilities and health workers, preferably within Nigeria. Experience supporting the implementation of maternal, newborn, child, adolescent health and nutrition interventions. Experience conducting Quality of Care assessments using national or recognized QoC standards and assessment tools. Demonstrated experience in establishing, strengthening, or supporting Quality Improvement Teams (QITs) and facility-level quality improvement initiatives. Experience applying quality improvement methodologies, particularly Plan-Do-Study-Act (PDSA) cycles. Demonstrated experience using QI tools such as Five Whys, Fishbone/Ishikawa analysis, process mapping, and other root-cause analysis approaches. Experience supporting health facilities to use routine health information and service delivery data for quality improvement and decision-making. Experience in Infection Prevention and Control (IPC), referral systems, clinical standards, and adherence to national guidelines. Previous experience working with State Ministries of Health, SPHCDA, LGAs, development partners, or other health-sector stakeholders will be an advantage. Technical Competencies

RMNCAEH and Nutrition service delivery at primary and secondary healthcare levels. National clinical standards, guidelines, protocols, and quality-of-care requirements. Quality Improvement methodologies and tools. Facility-level QoC assessment and performance improvement. Data quality assessment, data analysis, interpretation, and use. DHIS2 and other routine health information.
