We are seeking a compassionate, responsible, and professional Social Worker to provide support, guidance, and assistance to individuals, families, and communities facing social, emotional, financial, or other life challenges. The Social Worker will assess clients' needs, develop appropriate support plans, connect clients with relevant services, and advocate for their well-being.

Conduct assessments to identify clients' social, emotional, financial, and community needs. Provide counseling, guidance, emotional support, and practical assistance to clients. Develop and implement individualized support and intervention plans. Maintain accurate and confidential client records and case files. Connect clients with healthcare, educational, housing, employment, legal, and community resources. Monitor clients' progress and make appropriate follow-up referrals. Advocate for clients' rights, safety, dignity, and access to essential services. Work with families, community organizations, healthcare professionals, schools, and other service providers. Respond appropriately to cases involving vulnerable individuals, including children, elderly persons, persons with disabilities, and people experiencing hardship. Conduct home visits and community outreach when required. Participate in community development, awareness, and empowerment programs. Identify cases requiring specialized professional intervention and make appropriate referrals. Prepare case reports, progress reports, and other required documentation. Maintain professional boundaries and protect client confidentiality. Participate in team meetings, training sessions, workshops, and community programs. Stay informed about relevant social welfare programs, community resources, and professional standards.

Number of clients successfully assessed and supported. Quality and timeliness of case documentation. Client follow-up and engagement rates. Successful referrals and access to support services. Participation in community outreach and empowerment activities. Compliance with professional and organizational standards. Client satisfaction and service outcomes.

Requirements and Qualifications

Bachelor's Degree or equivalent qualification in Social Work, Sociology, Psychology, Community Development, or a related field. Professional certification or registration in Social Work is an advantage. Previous experience in social work, community development, counseling, healthcare, or a related field is preferred. Strong communication and interpersonal skills. Good listening, counseling, and problem-solving abilities. Ability to work effectively with people from diverse backgrounds. Strong organizational and record-keeping skills. Ability to maintain confidentiality and demonstrate professional ethics. Compassionate, patient, trustworthy, and emotionally mature. Ability to work independently and as part of a multidisciplinary team. Basic computer and documentation skills.
